Beginning July 2025, the Immunization Section initiated the process of updating the Texas Administrative Code (TAC), Title 25, Chapter 97, §97.62 and §97.64, which includes immunization exemption affidavit forms. DSHS now posts a blank affidavit form on the www.immunizetexas.org website for a person to download and submit to their child-care facility, school, or institution of higher education, including students enrolled in health-related or veterinary courses. The proposed changes to §97.64 clarify exclusions from compliance for medical and veterinary students, aligning with current practice outlined in statute.
Individuals will still need to notarize the form before submitting it to their child-care facility, school, or institution of higher education, including health-related or veterinary courses. The notarized forms remain valid for two years.
The rule change is effective for the 2025–2026 school year.

When Your Child is Ill
DO NOT SEND YOUR CHILD TO SCHOOL IF HE/SHE IS ILL OR HAS:
(Must be free of fever without the aid of medication for at least 24 hrs.)
(Unless has note from health care provider stating child is not contagious)
If your child is sick or injured at school, you will be called to pick him/her up. A note will be sent home for minor injuries. PLEASE BE SURE THE SCHOOL HAS YOUR CURRENT HOME AND WORK PHONE NUMBER ON FILE.
*Staying home when sick will help your child get well and protect classmates.*

- SEVERE SORE THROAT
- EYE INFECTION
- DIARRHEA
- VOMITING
- HEAD LICE
- FEVER
- SKIN RASH
